I cannot say enough wonderful things about this facility. The staff is incredibly compassionate, attentive, and truly treats residents like family. I felt it when the admission marketing team greeted me upon my arrival. From the nursing & therapy them everyone goes above and beyond to ensure comfort, dignity, and excellent care. The activity department—bring so much joy, creativity, and engagement to the residents every day. The variety of activities and the genuine enthusiasm of the team make a huge difference in residents’ quality of life. The facility is clean, well-maintained, and has a welcoming atmosphere. Communication with families is consistent and reassuring, which makes such a difference during challenging times. I am grateful for the care and kindness shown here—highly recommend to anyone looking for a top-quality skilled nursing facility.

San Francisco Post Acute is an facility that encourages wellness and health for the patient. The activity department was super involved with giving the patients a fun and safe space, with karaoke or pizza making. The nursing staff are polite and gentle with their care. The facility is clean, smells good, and well-kept.

SAN Francisco post acute care about the staff and their resident .place is well clean and properly managed .lets not forget about the activities, they have the best activities like pizza making , art and many more .activities director and other staff make sure all resident participate on activities .

I insisted my brother be taken to SF Post Acute because the last time he was there they didn’t discharge him until he could walk and do things around the house. I knew they would do the same this time. They are concerned with helping the patient and returning them home as functional people. I like that. This is the best rehab facility my brother has been in.

Thoroughly satisfied with my grandma’s experience at SF Post Acute. Friendly staff and clean environment. We felt at ease knowing she was in good care at this facility helping her heal and rehab.

Not good. Staff is way under and it shows. Patients are left for long periods and many with behavioral issues pose risks to other patients. Food is slop. Rooms are cramped and no room for even a chair. Just don’t place your loved one here. Only upside is PT.

Not only is the environment clean and enriching, the rehab team is one-of-a-kind here. The physical, occupational, and speech therapists work so hard to get their patients better, and genuinely care for them. I would highly recommend SF Post Acute to patients that have any physical impairments and want to improve fast. Thank you SFPA! 10/10‼️⭐️⭐️⭐️⭐️⭐️

I don’t remember how long I was actually there a month ? Maybe 6weeks. It was just after surgery on my spine for decompression. I can’t say enough about the staff there! They took care of me when I couldn’t take care of myself. I will always remember the staff as being committed and caring. As bad as I felt having to be waited on the staff made me feel cared about not cared for. Thank you all. I am still walking with my cane but I’m up and out and about all day now and it’s because of the care I received from the staff there. Wesley B. Law

This place is awful …they don’t clean the rooms they leave patients bed unchanged don’t feed patients on time or at all ..my mother was mistakenly sent here for physically therapy rehabilitation…she was to much of an alert patient…they like patients who are incompetent an incompasitated ppl who are on their way out this world they are only bout money

My mother was sent here after her release from her week-long stay at the hospital. She was sent here to rehabilitate, needing OT and PT. She was admitted on Monday and released on Wednesday of the same week. There was no way she could have spent 48 hours at this facility and then be ready for release (she was unsteady on her feet, had weak muscle control and needed constant monitoring). The facility only allows for more than one visitor in the common area and only one visitor at a time was actually allowed to visit with the patient in their room.

The Administrator at this facility is top notch. Very helpful to families and incredibly understanding. I’ve noticed these places tend to take on the tone of their administrator and you can tell by all the managers there and their caring attitudes.

I left this place this week and my experience of the place was not so good. I wanted to get help from the therapies because they described it as very helpful on the page and it is not true, the therapy is not every day and it only lasts in 15 to 20 minutes and being there you cannot force them to help you. For me it was better to be in the hospital because they treat you better than this place, there were even conflicts because my family came to visit me and Mrs. Lee did not want to let them pass, it came to a point where my daughter ended up crying with anger because she contradicts herself like that myself. My first week there was very disappointing, I don’t know if all places are like that but the only thing I know is that this place didn’t help me much with the therapies they think that complying with 15 minutes is enough and it’s not like that. It’s not about winning the day, it’s about getting the job done.

This is one of the best skilled nursing facilities in San Francisco. It is never easy to choose a SNF but if you are in that position, turn to SFPA! Their attention to each patients plan of care can be seen throughout the stay. It has recently undergone a remodel and the attention to patient care is prevalent including adding personal TV’s with WIFI access for each patient. The staff are friendly and will make you feel at home.

The Activities Director, Lee, Is one of the best in the business, her parties are always fun. She always has fun prizes, and good food. She goes out of her way to throw parties for residents and staff. She deserves a raise for being a great Activities Director and helping out with visitor screening. She help keep them entertained and safe. LOVE YOU LEE!!!! I MISS SFPA!!!!!

I do not typically write reviews however I know how important it is for families to find a suitable nursing home for their loved ones. My dad was admitted to SF Post Acute Care after a foot amputation. My dad suffers from chronic pain on top of PTSD and other comorbidities. I am a Registered Nurse and have been caring for my dad since I was 15 years old with the help of my mom. Burn out is real and I understand the stress caregivers have to endure. My dad’s condition is very complex, and requires patience and compassion. SFPA was able to provide my dad with that and he appreciates them greatly. This nursing home is communicative, supportive, and empathetic towards their residents. Huge shoutout to Ms. Jerilee, who is a nurse and the admissions director at SFPA. She calls me regularly, always providing me with updates about my dad. She truly cares about the residents and their families. Thank you Jerilee, I will never forget what you have done for me and my family. Here’s a breakdown of what I think makes a good nursing home. Cleanliness: SFPA is one of the most cleanest facilities I have ever seen. The rooms, communal areas, and nursing stations are spotless. This is so important for elderly patients due to their susceptibility for infection. Location/Parking: SFPA is located in the Mission right off the 280 highway. The location is easily accessible. I live in the Bayview and it takes me only 10 minutes to get their facility. Parking is limited on site however there is plenty of residential parking. About a 2 block walk, but parking is free and safe. Scheduling: Many nursing homes have very strict rules in regards to their visitation policy. This is to maintain the safety of their residents. They are very flexible with scheduling appointments, just call a couple days in advance. Do not forget your COVID vaccination record. Activities: They host activities in the communal area everyday. One time I visited, I saw residents singing karaoke with the staff. It was so sweet to watch and you can see how happy the residents were. Staff: Staff are extremely friendly and welcoming to families. I always had a pleasant experience with the nursing staff. Administration is beyond amazing. They built rapport with my dad during his stay and he talks greatly about the staff. He’s always sharing stories about the activities he participated in and the friends he made. I can’t thank San Francisco Post Acute enough for what they have done for my dad. I will continue to recommend SFPA to friends and family.

I have been a patient here for about 10 days and the staff are the most warm and caring professionals I have ever had take care of me. They have gone out of their way just to make my stay comfortable. All the way down from the Director to the CNA’s and everyone in between. The physical therapy is amazing and you can feel your progress with them. They have great meals with a new one each day. I wish I had more words to describe the facility but can only say they are marvelous. Thank you so much for my care. KTT I

If your looking for a care home for your loved ones SF Post Acute is the place. I have visited a few care homes in the Bay Area and by far SF Post Acute’s facilities was impressive. From the warm and inviting environment, the friendly and compassionate staff members, the services they provide to their patients and the amenities available exceeded my expectations. Everyone was extremely helpful and you can see that the patients are being well cared for. And that allows you to have peace at mind. Some places seem cold and uninviting at first glance, but not here. You are welcomed with smiles the moment you walk in. Thank you Lee for the warm welcome and for allowing me to visit your beautiful facility.
